Wayne County mental health providers failed to file required outpatient treatment plans in roughly two-thirds of court-approved cases last year, a WXYZ investigation found.

In 2025, treatment plan notices were filed in just 453 of 1,380 granted mental health petitions, or 33%.

Why it matters: Missing treatment plans make it difficult for courts to track patients' court-ordered care.

The revelations follow the September stabbing death of a downtown Detroit Chipotle employee, allegedly by a man whose provider failed to file his court-ordered treatment plan.

Zoom in: The Wayne County Probate Court sent warnings to 21 mental health providers earlier this month, demanding compliance or threatening consequences.

What's next: The Detroit Wayne Integrated Health Network is investigating the treatment provided to the stabbing suspect.
