Dimer Health, a virtual patient discharge company, raised $13.5 million in a Series A funding round, CEO Caroline Hodge tells Axios Pro exclusively.

What's next: The company expects to to start conversations to build relationships in preparation for the next round, Hodge says. 

Follow the money:  Team8 and Bill Ackman's Table Management led, with  Silver Circle and TechAviv participating. 

The round gives Dimer approximately three years of runway and brings its total capital raised to $19.5 million.

How it works: The Florham Park, New Jersey-based company pairs virtual clinicians with AI agents to help patients navigate hospital discharge.

Its agents answer patient questions, assist with care plans and schedule follow-up appointments.

Zoom in: Dimer's current business is roughly an even split between value-based care arrangements, directly contracted with payors. and fee-for-service.

The company is eyeing more risk-bearing contracts and plans to further automate and personalize the care its patients receive.

What they're saying: While health systems are important to patients' care, Hodge says those resources don't always extend to patients after a stay.

"The current system and the current infrastructure doesn't allow for those resources to extend to the patient in their home," Hodge says.
