Inbound Health, a hospital-at-home startup that had raised over $50 million, shuttered Monday, per an audio recording Axios Pro has obtained.

Why it matters: The closure underscores mounting uncertainty around the hospital-at-home sector.

What they're saying: "Unfortunately, the board, particularly our investors on the board, did not approve of us moving forward with a process to bring in new capital and keep the company solvent," Inbound CEO Dave Kerwar said on the recording of the company's last all-hands meeting.

Per Kerwar, the company did not have enough capital to sustain itself beyond Q1 2026.

One high-level Inbound employee told Axios Pro that the Minneapolis-based company was growing too slowly.

Driving the news: A federal hospital-at-home waiver fueled venture funding into home care startups — lifting Medicare's 24/7 onsite nursing requirement and allowing hospitals to deliver acute care at home — but it expired at the end of September.

Inbound's board and investors doubted the company could raise new funds in January or February as they "had no confidence in the extension of the [hospital-at-home] waiver," Kerwar said during the call.

"This was entirely due to regulatory uncertainty as demonstrated by the recent government closure, and the lack of a long-term extension of the AHCAH Waiver Program," Kerwar said in an email to Axios Pro.

Inside the room: Minneapolis-based Inbound was working on a health system "consortium deal" that would have included at least two new partners, Kerwar said during the call. 

The deal would have had two or more health care systems give Inbound a capital infusion in exchange for equity, per the high-level employee source.

Nevertheless, Inbound's investors and board "did not feel like that was enough capital to keep us going, you know, well beyond sort of Q1," Kerwar added. 

Catch up quick: Inbound's cap table includes Flare Capital Partners, McKesson Ventures, HealthQuest Capital, Allina Health System, and Gershon Capital.

Inbound, Flare, HealthQuest and Allina Health did not respond to a request for comment.

What's next: All employees' last day of employment is Monday, and no employees will be receiving severance.

How it worked: Inbound combined biometric monitoring, digital surveillance, in-home nursing and therapy, and virtual visits with hospitalists and geriatricians to care for patients in their homes.

Zoom out: The hospital-at-home sector faces regulatory challenges and financial pressure from investors and board members due to:

A lack of long-term reimbursement clarity for the program.

Regulatory hurdles in developing standardized protocols for program evaluation.

A lack of systematic evidence on the programs' cost-benefit trade-offs.

Case in point: When hospital-at-home companies DispatchHealth and Medically Home merged in June, a leaked 498-page document suggested a dramatic loss in valuation for Medically Home, per Axios reporting.

The big picture: "There's just been so much regulatory volatility it makes it hard for any business, any health plan, to plan strategically," says Constantinos Michaelides, medical director of hospital-at-home programs for UMass Memorial Health.

The bottom line: "Hospital-at-home is not a stable model," says Ann Richardson, a health care consultant with experience in leadership roles at health systems.
