Nest Health, a family-focused value-based care company, raised $22.5 million in a Series A funding round, CEO Rebekah Gee tells Axios Pro exclusively.

Zoom in: The CEO says its clinicians see an average of two and a half patients per home visit. 

How it works: Nest provides family-focused value-based care arrangements. Its care teams include medical assistants, who it calls family advocates, along with nurse practitioners and physician assistants.

In addition to completing annual wellness visits, the company's care teams also help patients with medical needs like administering vaccines or drawing blood.

After an initial visit, Nest can complete other care via telehealth — such as behavioral health visits or urgent care — with the aim of lowering costs and reducing patient trips to emergency rooms.

Zoom in: Nest currently operates in parts of Arizona and Louisiana, covering approximately 30,000 users.

Follow the money: Venture capital firms 8VC, Blue Venture Fund and Amboy Street Ventures co-led the round. Cox Enterprises' venture firm Socium Ventures participated.

The round brings the company's total raised to $43 million.

The company plans to raise again by the end of 2027.

What's next: Nest plans to add approximately 40,000 users by the end of 2027.

Nest currently uses AI to help identify at-risk members and draft post-visit care plans. The company also uses AI to help identify the best times to reach members and assist with phone calls.
